We are seeking an experienced LVN Case Manager to join our Valencia, CA corporate office. This role is critical in leading patient care coordination efforts, ensuring clinical documentation compliance, supporting scheduling operations, and enhancing patient outcomes while upholding California state and federal healthcare laws.

Position Overview

The LVN Case Manager supervises Patient Care Coordinators, Schedulers, and Virtual Support Staff. This individual works collaboratively with fellow Case Managers, providers, and administrative staff to ensure effective mobile wound care service delivery, regulatory compliance, and an outstanding patient experience across Omni Wound Physicians' multi-state operations.

Key Responsibilities

Clinical Operations and Regulatory Compliance

Oversee the coordination of mobile wound care services ensuring timely, high-quality patient visits at SNFs, assisted living facilities, and private residences.

Collaborate with physicians, nurse practitioners, and physician assistants to manage urgent patient needs including gastrostomy tube replacements, abdominal KUBs, and lab orders.

Ensure compliance with California Department of Public Health (CDPH) regulations, Skilled Nursing Facility standards, mobile healthcare regulations, and CMS Conditions of Participation.

Maintain strict compliance with HIPAA, California Consumer Privacy Act (CCPA), and Protected Health Information (PHI) confidentiality requirements.

Team Leadership and Development

Supervise, train, and evaluate assigned Patient Care Coordinators, Schedulers, and Virtual Support Staff.

Foster a positive, collaborative, and compliance-driven team environment.

Ensure adherence to Omni Wound Physicians policies, clinical guidelines, and regulatory requirements.

Documentation, Authorization, and Compliance Tracking

Ensure accurate, timely documentation of all clinical communications, treatment plans, referrals, and patient authorizations in the EMR (Wound Ally).

Conduct routine audits to ensure documentation accuracy and regulatory compliance.

Coordinate with billing and compliance departments to support clean claims and minimize denials.

Scheduling and Resource Management

Manage scheduling workflows to optimize provider availability and ensure patients receive timely care.

Proactively resolve scheduling conflicts to maintain continuity of care.

Secure required patient consents and maintain documentation in compliance with California consent-to-treat regulations.

Patient Care Coordination

Manage patient care from referral through discharge, ensuring seamless communication between providers, facilities, and patients' families.

Coordinate care with SNFs, assisted living communities, and home settings as appropriate.

Monitor hospitalization lists and manage patient transitions post-discharge.

Compliance and Continuous Improvement

Ensure ongoing compliance with HIPAA, CCPA, CMS, CDPH, and California labor laws applicable to healthcare service delivery.

Actively participate in internal audits, survey preparations, and quality improvement initiatives.

Identify and recommend workflow enhancements to improve clinical outcomes and operational efficiency.

Remain flexible and responsive to urgent clinical or operational requirements.

Physical Requirements

Ability to sit or stand at a workstation for extended periods.

Manual dexterity to operate office equipment (keyboard, phone, copier, scanner).

Ability to lift and carry up to 20 pounds as needed (files, office supplies).

Ability to travel locally to provider meetings, trainings, or facilities if required.

Technical Requirements

Proficiency in Electronic Medical Records (EMR) systems (Wound Ally preferred or equivalent systems).

Strong working knowledge of Google Suite (Gmail, Drive, Docs, Sheets, Calendar).

Proficiency in Slack for real-time team communication and collaboration.

Basic troubleshooting skills for online systems and virtual collaboration platforms (Zoom, Microsoft Teams).

Ability to manage and document across multiple digital platforms simultaneously.

Qualifications

Active and unrestricted LVN License in the State of California (required)

Graduate of an accredited vocational nursing program (required)

Associate's degree in Nursing, Health Administration, or a related field (preferred)

Bachelor's degree in Nursing, Public Health, or Healthcare Administration (a plus, but not required)

Minimum 3 years of clinical healthcare experience (preferably in wound care, home health, SNF, or mobile healthcare settings)

Minimum 1 year of leadership, case management, or supervisory experience preferred

Strong knowledge of HIPAA, PHI, CCPA, California healthcare compliance, and SNF/mobile care workflows

Proficiency in clinical documentation, EMR systems (Wound Ally preferred), and healthcare coordination

Bilingual is a plus but not required.

On-site at Omni Wound Physicians' Corporate Office:
Valencia, CA 91355.

Standard business hours: Monday through Friday, 8:00 AM - 5:00 PM.

Flexibility required for occasional after-hours tasks based on operational needs.
