• The LVN Case Manager supervises a team of Patient Care Coordinators and Schedulers, overseeing clinical processes and ensuring efficient patient care coordination.
• Responsibilities include managing schedules, developing care plans, resolving escalated issues, and maintaining accurate documentation.
• Acts as a liaison among clinical staff, physicians, and administrators to optimize outcomes, ensure compliance, and foster collaboration.
• Coordinates procedures, lab orders, medication management, and maintains EMR accuracy.
• Provides team leadership, monitors workflows, and supports continuous improvement efforts.
• Requires LVN license, 1-year case management and wound care experience, and is based in Valencia, CA.
• This full-time role involves an 8-hour weekday schedule, compensation starting at $22/hour.
