The LVN Case Manager will oversee a dedicated team of Patient Care Coordinators, Schedulers, and manage all clinical aspects related to their team's patient care. This individual is responsible for ensuring the efficient coordination of patient services, overseeing scheduling, managing patient care plans, and fostering a collaborative work environment. The Case Manager will also act as a liaison between clinical staff, physicians, and administrative personnel to ensure optimal patient outcomes.

Key Responsibilities

Team Leadership & Management :

Supervise and support the daily activities of Patient Care Coordinators and Schedulers.

Provide guidance, training, and performance management for team members.

Ensure team members adhere to company policies, clinical protocols, and regulatory

requirements.

Clinical Oversight:

Oversee the coordination and management of clinical processes to ensure timely, quality

care for patients.

Collaborate with physicians and clinical staff to manage patient care plans and treatment

schedules.

Act as the clinical lead for the team, resolving escalated patient care issues and making

clinical decisions when necessary.

Scheduling Workflow Management:

Monitor and optimize scheduling workflows for patient visits, procedures, and follow-ups.

Ensure the appropriate allocation of resources to meet patient needs in a timely manner

avoiding missed visits

Address and resolve any scheduling conflicts or delays in care.

Patient Care Coordination:

Coordinate care for patients from referral through treatment and follow-up.

Ensure that patient information is accurately documented and communicated between

departments.

Support the team in delivering exceptional customer service and maintaining strong

patient relationships.

Collaboration & Communication:

Serve as a key point of contact between physicians, clinical staff, and administrative

teams.

Work with other department heads to continuously improve patient care and operational

efficiency.

Communicate effectively with patients, families, and external partners to ensure

continuity of care.

Continuous Improvement & Compliance:

Identify opportunities for process improvements to enhance clinical outcomes and

patient satisfaction.

Ensure compliance with all clinical guidelines, company policies, and regulatory

requirements (HIPAA, CMS, etc.).

Day to Day:

Coordinate abdominal KUB's and Gastrostomy tube replacements for urgent needs and

routine replacements

Coordinate lab orders

Team building and oversight.

Complaint management.

Appropriate follow up with pharmacies on medication orders.

Coordinate with scheduling to manage the provider's schedule.

Maintaining and updating EMR: Wound Ally with the highest level of accuracy as needed

Management/leadership of team (schedulers, patient care coordinators, virtual staff)

Monitor patient hospitalization lists.

Maintain and update schedules, calendars, and agendas.

Confirming appointments and obtaining patient's consents.

Sending out reminders of scheduled appointments.

Answering queries via email and phone.

Performing other administrative tasks when required.

Ensure medications, supplies, and Durable Medical Equipment orders are processed

Pay: From $22.00 per hour
