LPN Rehab Unit Coordinator

We are seeking an experienced Licensed Practical Nurse (LPN) for the role of Rehab Unit Coordinator in a 20-bed post-acute rehabilitation setting.

•  Lead and mentor nursing staff, ensuring high-quality, patient-centered care and effective discharge planning.

•  Coordinate with healthcare professionals to develop and implement comprehensive rehabilitation plans.

•  Oversee care delivery, monitor patient progress, and foster continuous improvement.

•  Ensure compliance with regulations and facility policies.

•  Qualifications include a valid Indiana nursing license, post-acute rehab experience preferred, strong leadership and communication skills, and a passion for team development.
