1. Demonstrates the skills and judgments to implement the medical plan of care, nursing interventions and procedures as necessary for the care of the patient, with the guidelines of his/her licensure. 2. Completes monthly client progress notes and vital signs. 3. Administers and records medications as ordered by the clients' physicians in accordance with the standards of practice. 4. Collaborates and communicates on a regular basis with the Program Director/RN and members of the Adult Day Health team. 5. Assists with feeding, toileting, ambulating, transporting and other tasks which may be required. 6. Assesses and identifies changes in clients' condition and needs and reports to the Program Director/RN. 7. Assists as necessary in the delivery of other required program services. 8. Maintains confidentiality of client/employee information. 9. Maintains confidentiality of administrative, marketing and clinical business practices, policies, procedures, techniques and plans. 10. Communicates to the Program Director/RN all information necessary to promote and maintain the highest quality and continuity of client care 11. Maintains environmental safety for the client and the staff. 12. Accurately and safely utilizes all equipment and supplies appropriate and necessary for the care of the clients. 13. Follows appropriate infection control techniques. 14. Consistently functions as a member of the program team in a positive manner. 15. Ensures adequate nutrition and assists in the supervision of meals and snacks to ensure compliance with each client's prescribed diet. 16. All other duties as assigned. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA's commitment to CARES: compassion, accountability, respect, excellence and service. Current LPN license in MA CPR certification required One year of recent experience in an acute facility. Experience with older adults preferred.
