Overview:

We are looking for an AIM Clinical Coordinator LPN to join our team. This position will directly report to the Clinical Manager AIM and is responsible for coordinating and managing all day to day business affairs and other clinical activities related to the administration of the Advanced Illness Management (AIM) and Palliative Care department.

  • Schedule and manage frequency adherence, clinical coordination, medical record maintenance function, and marketing function
  • Field incoming calls from partners and patients
  • Manage Visit Freq compliance, scheduling PRN visits, post hospitalization follow up with patient/family-expediting discharge home with AIM communication with partners, schedule post hospitalization visits.
  • Order labs, DME, imaging. Coordinate results with NPs, enter telephone encounters in partner EMR, enter routine visit med reconciliation in partner EMR.
  • Ensure transitions to HSP referrals to CTC for care type.
  • Assist in appropriate identification of HSP eligible pts. identifying HH providers that can accept HH referrals from PCC.
  • Identify and communicate local partner office for lab drop off
About You:
  • Bachelor degree in Health Care administration, nursing or related degree
  • Minimum of three years health care delivery or related business experience required
  • Experience in Part B billing setting preferred
  • LPN License required
  • Experience with Computer applications and Electronic Medical Records

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.


Salary Range - $21.60-$25.25/hr
