Job Summary: As a Clinical Coordinator, you will contribute to the success of our organization by ensuring the highest quality of patient care. Your role will include leading teams, maintaining organized and efficient systems, tracking and enhancing operations, and ensuring continuity of care for all patients. In addition to direct team leadership, you will also support the clinical supervisor in various administrative tasks.
Key Responsibilities:
- Team Building & Leadership: Build and lead teams to provide the highest quality of life for our patients.
- System & Communication Management: Maintain organized, effective systems to ensure the continuity of quality patient care.
- Operational Evaluation: Evaluate agency operations, maintain tracking systems, and identify ways to improve workflow and productivity.
- Medical Record Management: Assist staff in maintaining accurate and current medical records, including utilization reviews.
- Non-Clinical Support: Support the clinical supervisor by handling non-clinical phone inquiries.
- Administrative Tasks: Notify the supervisor of any administrative issues requiring attention, process and distribute daily mail, maintain inventory, and clean equipment as needed.
- Prevent Hospitalization: Implement actions to reduce avoidable hospitalizations and ER visits.
- Ongoing Education: Attend training, seminars, and other learning opportunities to improve care and knowledge.
- Positive Communication: Communicate effectively with patients, caregivers, physicians, families, visitors, and other team members.
- Promote Elara's Philosophy: Advocate for Elara's mission and policies to ensure the highest standards of care.
- Confidentiality: Uphold patient and staff privacy and confidentiality in accordance with HIPAA regulations.
- Additional Duties: Perform other duties and projects as assigned.
What Is Required:
- 1+ year of experience in a hospice or home health care setting.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing. OR Graduate of an accredited Certificate, Diploma.
- Proficiency with medical terminology.
- Experience working with a clinical management team.
- Proficiency with office equipment, including computers, fax, and copy machines.
- Experience with Medicare and Medicaid guidelines (preferred).
