LPN Licensed Practical Nurse

•  Requires certification from an accredited Physical Therapy program and current Connecticut Physical Therapy Assistant license.
•  Minimum two years' experience in inpatient, outpatient, or home care (excluding mental health settings).
•  Must have strong knowledge of physical therapy principles, excellent communication, organizational, interpersonal, and independent decision-making skills.
•  Valid driver's license, reliable car, and auto insurance required.
•  Delivers physical therapy treatments per plan established by a Physical Therapist, coordinates with healthcare teams, maintains accurate documentation, and ensures quality care in a home care setting.
