Overview:
Responsible for providing skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse. Must assess, plan, implement and evaluate patients needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Qualifications:
Education: Graduate of an accredited practical nursing program; Consideration will be given for comparable education experience and successful licensure.
Licensure: Current LPN license in the state of practice. Must be a licensed driver.
Certification/Registration: Possesses and maintains current CPR certification.
Other skills & knowledge:
Complies with accepted professional standards and principles.
Understands Hospice philosophy; issues related to death/dying and grief.
Complies with Unity Health Hospice Health Status requirements.
Has ability to provide good written documentation in a timely manner.
Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
Employee has significant impact on patient welfare. Effects or errors in determining patient condition or administering medication could be critical.
Employee is frequently to continuously alone with patient and must be able to handle emergencies without assistance.
Employee must be able to assist falling or fallen patient; must be able to perform CPR alone.
Individual homes may present poor lighting situations, exposure to pets and other allergens, pest infestations and poor cleanliness.
Employee may be exposed to irate and abusive patients and families.
Employee must be able to assess home situation for possible patient abuse.
Employee may be required to drive at night and/or in hazardous conditions; must be able to follow written/verbal instructions and read a map.
Employee must independently schedule patient visits in timely and efficient manner; checking in with office daily to update schedule.
Employee has regular contact with agency staff members, physicians, hospital or medical center staff, general public.
Employee regularly handles confidential information.
- Preferred Experience: Prefer one year experience in a related field.
