RESPONSIBILITIES: The LPN shall be directly supervised by the Director of Health Services, DON, Clinical Manager, or equivalent as identified in the table of organization.
Person-centered care
Strong documentation skills
Medication administration, updating orders and day to day care coordination
Effectively communicate with Physicians, Supervisors, Guardians, DSP and Midwest Programming Department.
Dietary support and monitoring
Relevant education and training based on the individuals care plan
Complete Quality Assurance reviews in the group homes
Provide on call consultation to Midwest DSP and Programming Department during normal business hours and participate in On Call Rotation for after hours as scheduled by Midwest Health Services Nursing Department.
Attend Staff and ISP meetings if medically necessary
Travel to homes within the assigned region, to include Wayne County, Ashland County, Richland County.
Complete Training to become an American Heart Basic CPR and First Aid Instructor and Instruct classes as scheduled.
Complete all other duties as assigned.
Job Qualifications:
Certificate in nursing from an accredited nursing school
Licensed as LPN as issued by the State of Ohio-Registered on the State Nurse Registry
Possess skills to complete LPN duties
Required:
High school diploma, High school transcripts, GED, or equivalent
Copy of Nursing License: LPN
BLS for Healthcare Provider
American Heart or Red Cross
Driver's License
Two professional reference letters
Auto insurance
Social Security Card
