We are seeking a part time Nurse Practitioner (NP) that will play a vital role in delivering comprehensive primary medical care to participants of the PACE program in collaboration with PACE Physicians and the Interdisciplinary Team (IDT). Responsibilities include conducting assessments, developing and implementing individualized care plans, evaluating and treating participant health concerns, and providing education to participants and caregivers. The NP oversees daily clinic operations, performs rounds in contracted nursing homes, and ensures timely communication of participant health status to the IDT. This role requires strong clinical judgment, adaptability, and the ability to work independently in a dynamic, team-oriented environment focused on supporting frail older adults in maintaining independence and quality of life.
Mon - Friday. 20 hours per week. Part time hours
ANCC/AANP Board Certified - REQUIRED Active PA license - REQUIRED 5-7 years of NP experience required preferably in long-term care and a minimum of one (1) year working with the frail or elderly population BLS - REQUIRED DEA Registration - REQUIRED Clean/Clear malpractice - HIGHLY PREFERRED Certification as a Nurse Practitioner - REQUIRED
