Description of Work Assignment : Will complete all Intakes and Annuals in OTP. Will also complete dose evaluations, medical assessments and illicit groups with patients. Location of Work Assignment: 1510 W Franklin Street, Evansville, IN 47710 Breakdown of Clinic Days/Sessions and/or OR Days: Monday-Friday 6:00am-12:00pm Schedule of Days/Times Needed To Work/ Shift Hours: Monday-Friday 6:00am-12:00pm On call?
