Practice Setting

Schedule
- Monday-Friday, 8AM-5PM

Requirements
- MDs only-board-certified Family Medicine or Internal Medicine
- Active Illinois license
- DEA required and 12+ months of experience
- Fluency in Spanish is highly advantageous
