- Schedule: Monday - Friday 8am - 5pm with an hour rest period
Responsibilities and Duties
- Conduct health histories and physical exams
- Provide patient teaching tailored to individuals
- Order, interpret, and follow up on laboratory tests
- Travel for home health visits as needed
- Perform selected medical procedures
- Document patient care comprehensively
- Initiate emergency measures when necessary
- Evaluate, review, and modify orders for medications and diagnostic studies
- Assess and coordinate patient care daily
- Request consultations and collaborate with other medical services
- Act as a liaison with healthcare professionals, providing education and support
- Participate in staff meetings and contribute to patient care planning
- Support the PACE mission through ethical clinical practices
Additional Information
- Must have at least 1 year of experience in geriatric care
- Basic Life Support (BLS) certification required
- Licensed to practice in North Carolina
