Therapist MSW - Hospice Newport Beach, CA LOCALS ONLY - No travelers accepted Facility: Hoag Memorial Hospital Presbyterian - Hoag at Home Shift: Monday - Friday, 8:30 AM - 5:00 PM Duration: 13 weeks Job Description The Therapist MSW provides comprehensive psychosocial assessments, care planning, and intervention for patients receiving home-based hospice care. This role supports patients and families in coping with illness, grief, and other psychosocial stressors. Responsibilities: Conducts biopsychosocial assessments and develops individualized treatment plans Provides counseling, crisis intervention, and emotional support for patients and families Coordinates care with interdisciplinary teams and connects patients to community resources Facilitates support groups and educational workshops as needed Documents all interventions and maintains accurate records Requirements: Education: Master's Degree in Social Work (MSW) from an accredited program Experience: Minimum 1 year of hospice experience required, Experience with culturally diverse populations preferred Certifications & Licensure: BLS (AHA) required, LCSW or LMFT preferred Other Requirements: Must be comfortable traveling to patient homes (4-6 visits per day) Strong knowledge of mental health, therapy, and social services Work Location Hoag at Home - 4041 MacArthur Blvd, Suite 100, Newport Beach, CA 92660
