MedAdventures is seeking a dedicated and skilled Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Manchester, CT. Responsibilities:
- Evaluate and treat patients with physical impairments in a Skilled Nursing Facility (SNF) setting.
- Develop and implement individualized treatment plans tailored to patient needs.
- Educate patients and their families on exercises and strategies to continue progress outside of therapy.
- Maintain accurate patient records and document treatment outcomes.
- Collaborate with the multidisciplinary healthcare team to ensure optimal patient care.
Qualifications:
- Current state licensure as a Physical Therapist (PT) in Connecticut.
- Experience in a Skilled Nursing Facility (SNF) setting preferred but not required.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team-oriented environment.
- Commitment to delivering exceptional patient care.
