We are seeking a skilled and motivated Physical Therapist (PT) to join a supportive Home Health team for a 13-week contract assignment. This is a full-time, day-shift role with guaranteed hours, focused on delivering high-quality, in-home patient care. Candidates must have recent home health experience and a strong commitment to patient outcomes.
Key Details: - Start Date: May 19
- Duration: 13-week contract
- Schedule: Monday-Friday, 5x8s (8 hours/day)
- Guaranteed Hours: 40/week
- Overtime: Not offered
- On-Call: No call requirement
- Floating: Must be flexible and willing to float as needed
- Time Off: All RTO must be submitted and approved in advance
Required Credentials: - Active Physical Therapy License (State-specific)
- BLS Certification (Basic Life Support)
- Home Health experience is required
- OASIS experience highly preferred - candidates must be comfortable using it regularly
- HCHB (Homecare Homebase) experience preferred - training available if needed
Productivity Expectations: - Caseload Target: 25 productivity points/week
- May go up to 30 points with routine visits
- Documentation must be timely and accurate, with use of electronic medical records
Additional Information: - Interviews will be conducted by the client prior to placement
- No holiday or on-call coverage required for this role
- Must be adaptable, organized, and self-directed in managing home visits
- Reliable transportation is required
