Job Description:
We are seeking a skilled and compassionate Physical Therapist to coordinate and provide restorative and rehabilitative Physical Therapy services to participants both at the center and in their homes. Collaborating closely with the Primary Care Physician, the rehabilitation team, and other Interdisciplinary Team (IDT) members, the Physical Therapist plays a crucial role in enhancing participant independence, safety, and functional abilities.

Essential Job Duties:

• Conduct Physical Therapy assessments in the facility and in participants' homes to identify care needs, preferences, and goals.
• Actively participate in IDT meetings to develop individualized care plans.
• Deliver and document Physical Therapy interventions in line with physician orders and care plans.
• Provide active and passive exercise, muscle re-education, gait training, and functional training of Activities of Daily Living (ADLs).
• Administer treatments such as massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, offering training, instruction, and guidance to implement Functional Maintenance Programs (FMP).
• Train and educate staff, participants, and caregivers/families on the use and maintenance of therapeutic equipment.

Job Requirements:

• A Bachelor's degree in Physical Therapy and at least five (5) years of Physical Therapy experience.
• A Master's degree or Doctorate in Physical Therapy is preferred.
• A valid California Physical Therapy License or proof of License eligibility.
• Two (2) years of Physical Therapy experience in a clinical setting with a focus on frail or elderly populations is preferred.
• Foreign Physical Therapy degrees must be evaluated to meet U.S. equivalency standards.
