Pride Health is hiring a RN Case Manager to support our client's medical facility based in NC. This is a 6+ month assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization.

Location: Lexington, NC 27292
Duration: 6+ months Contract (Possibilities of extension)
Schedule: Mon - Fri 8:00 am-5:00 pm (5 days/wk).
Job Responsibilities:
- Ensure accurate, complete documentation and coding.
- Communicate and collaborate with interdisciplinary care team.
- Support care coordination activities and benefit determinations.
- Monitor and respond to changes in patient condition in collaboration with APC and facility.
- Assist with therapy and medication coordination.
- Attend facility-based meetings and family conferences.
- Schedule and participate in team meetings and case presentations.
- Support training, tracking, and quality measure compliance.
- Conduct and assist with in-services for selected staff.
- Assist with nursing rounds, assessments, and documentation.
- Write provider verbal orders per state and facility regulations.
- Manage patients during acute episodes or chronic condition exacerbations.
- Support care during short-term sub-acute rehab episodes.
Requirements:
- Active and unrestricted license in the state which you reside, or ability to obtain by start date.
- Associate degree or higher in Nursing (RN)
- Certified in BLS.
- Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment.
- Access to reliable transportation.
