Clinical Coordinator - Rehabilitation Center Full-Time Monday-Friday 7a-3:30p 1 Weekend rotation a month Position Summary: The Clinical Coordinator plays a key leadership role within the rehabilitation team, supporting nursing staff and ensuring high-quality, resident-centered care. This position focuses on improving clinical outcomes by coordinating care plans, collaborating with providers, and guiding daily clinical operations. Pay: Competitive and based on clinical and leadership experience. Additional compensation provided for hours worked after 3:00 PM and on weekends. Responsibilities: Coordinate clinical services for all residents in the rehab unit Oversee individualized care plans and ensure follow-through by nursing staff Assign staff based on care needs and ensure effective communication Provide emotional and social support to residents and families Assist nurses with bedside care and participate in audits and floor rounds Foster a culture of compassion, responsiveness, and teamwork Qualifications: Active NYS RN or LPN license required Minimum 1 year of experience in long-term or acute care At least 1 year of formal healthcare leadership experience Rehabilitation experience strongly preferred Current CPR certification required Strong communication and leadership skills Must meet NYS Department of Health immunization requirements
