• CA RN license.
• Active BLS certification.
• Outpatient experience.
• 2+ years of experience within Oncology. Summary: Our client is looking to hire a Nurse Coordinator for a 6-month contract to hire position onsite in Palo Alto, CA. The Nurse Coordinator will be a part of the Oncology float pool meaning you will cover different oncology clinics at the Palo Alto location. You will assist with coordination of care of patients including setting up referrals, triaging, etc. You will be working in an outpatient clinic setting. As an integral member of the care team, you will collaborate with other health care professionals to ensure continuity of care and coordination of a wide variety of patient care services. Responsibilities:
• Coordinate patient care by assessing needs and implementing appropriate nursing interventions.
• Administer medications and perform procedures such as IV insertion and catheterization.
• Maintain accurate patient records using EHR systems and ensure compliance with healthcare regulations.
• Collaborate with healthcare teams to develop and execute care plans.
• Educate patients and families on health management and treatment options. Monitor patient progress and adjust care plans as necessary.
