Job Description:
Parking Reimbursement: $250/ Every Month in the first pay Cheque
Schedule & Unit Details:
40 hours/week, 4 10-hour day shifts with no fixed day off.
Every 6th weekend; no major holidays.
Work on Menino 7 West - a Med-Surg/Telemetry unit caring for stroke and general med/surg patients.
Required Experience:
Minimum 3 years of Med/Surg experience.
Strong understanding of current Med/Surg practices, acute inpatient care, physical/mental impairment assessments, and treatment planning.
Preferred Experience:
Over 5 years in Utilization Management (UM) and Case Management (CM).
Experience with EPIC EMR is highly desirable.
Certifications:
Required: BSN
Preferred: CCM or a related certification
Role Overview:
Case Manager will oversee care coordination for 18-22 patients using InterQual and Medicare criteria to assess medical necessity.
Responsibilities include planning, evaluating care options, managing length of stay (LOS), and ensuring cost-effective outcomes through resource optimization.
