Case Manager - MS/T Unit, Menino 7 West (Stroke & Med/Surg Patients) Schedule: AM Shift - 4x10-hour days (40 hours/week) Required Experience: Minimum 3 years of experience Strong knowledge of current Medical-Surgical (MS) practice patterns and acute inpatient care Proficient in methods for assessing physical and mental impairment Understanding of illness characteristics and expected treatment plans Familiarity with insurance benefits, plan contracts, eligibility issues, and requirements Preferred Experience: 5+ years in Utilization Management (UM) and Case Management (CM) Experience with EPIC electronic medical record system Required Certification: MA RN License BSN (Bachelor of Science in Nursing) Preferred Certifications: CCM (Certified Case Manager) or a related certification Department Details: The Case Manager (CM) will work collaboratively to assess, plan, implement, monitor, and evaluate care plans for 18-22 patients on the Menino Campus. Responsibilities include: Meeting patient needs while managing length of stay (LOS) Promoting efficient use of healthcare resources Determining medical necessity and appropriateness of admission using InterQual screening tools and inpatient lists from Medicare Utilizing effective communication and available resources to drive quality, cost-effective outcomes
