Summary:
The Clinical Care Coordinator supports the Clinical Manager in supervising staff, ensuring compliance, and promoting patient safety in home health care. This role involves training, oversight, and case management to improve quality outcomes.
Key Responsibilities:
- Supervise and guide nursing staff.
- Develop KPIs and ensure compliance.
- Conduct training and oversee patient care.
- Address policy updates and quality improvements.
Qualifications:
- RN license in the state of practice.
- CPR certification and home health experience (1+ year).
- Knowledge of OASIS E1, wound care, VAC care, ostomy care, and IV administration preferred.
Requirements:
- Valid driver's license, auto insurance, and background check.
- Ability to perform fieldwork as needed.
