Position Highlights:
- Conduct home-based clinical visits to manage chronic conditions and drive improved outcomes.
- Collaborate with interdisciplinary teams including pharmacists, care managers, and social workers.
- Utilize technology to support remote assessments, documentation, and care coordination.
- Flexible scheduling and independent fieldwork balanced with remote responsibilities.
- Full-time W2 role with mileage reimbursement and comprehensive benefits.
