As a Home Health Physical Therapist, you will play a crucial role in helping patients regain mobility and independence in the comfort of their own homes. Reporting to the Rehabilitation Director, you will use your expertise in anatomy, physiology, and medical terminology to develop personalized care plans. Your ability to assess functional abilities, implement effective treatment strategies, and coordinate discharge planning will ensure optimal patient outcomes. Responsibilities Perform comprehensive patient evaluations to assess physical condition and functional abilities Develop and implement customized treatment plans based on patient goals and needs Utilize therapeutic interventions such as manual therapy, therapeutic exercise, and modalities to promote recovery Monitor patient progress and adjust treatment plans as needed to ensure the best outcomes Educate patients and caregivers on home exercise programs and lifestyle adjustments for continued progress Collaborate with healthcare professionals to coordinate patient care, including discharge planning and follow-up services Maintain accurate and timely documentation in compliance with regulatory standards Stay up to date with advancements in physical therapy and engage in ongoing professional development Qualifications Active Physical Therapist license Strong assessment and patient care skills In-depth knowledge of anatomy, physiology, and rehabilitation techniques Experience with home health, geriatrics, and outpatient care preferred Proficiency in medical terminology and treatment planning Ability to work independently and manage a flexible caseload
