Physical Therapist Assistant - Home Health

Key Responsibilities:
- Assist physical therapists in providing physical therapy treatments and procedures as outlined in the care plan.
- Observe and document patients' status before, during, and after therapy, reporting progress and concerns to the supervising physical therapist.
- Help patients perform specific exercises and use therapeutic equipment safely in the home environment.
- Educate patients and family members about exercises and activities to continue between visits.
- Adapt interventions as needed within the scope of the treatment plan and state laws.
- Ensure all documentation is accurate and completed in a timely manner according to agency and regulatory standards.
Work Environment:
- The role is based in patients' homes, requiring travel within the community and adaptability to various home settings.
