Licensed Social Worker

The medical social worker implements care standards for social work services, reporting to the Clinical Operations Manager.

•   Assesses patients' psychosocial status and communicates findings to nursing staff.

•   Conducts social evaluations and plans interventions.

•   Maintains clinical records and provides information and referrals for patients and families.

•   Acts as a liaison with community agencies and collaborates with healthcare personnel.

•   Participates in care planning and discharge processes.

•   Requires a Master s or Doctorate in a related field, Illinois social work licensure, and CPR certification.

•   Minimum one year of home health experience is necessary.

•   Must have strong communication skills and reliable transportation.

•   Job involves various settings with potential exposure to health risks.
