Under the supervision of the Health Department Manager-Registered Nurse (RN) of the site, and the Program Director for the Women, Infants, and Children (WIC) Division, the Licensed Practical Nurse (LPN) would, provide WIC Certifications for clients of the site. Certification reviews would include health history, anthropometrics, hemoglobin or hematocrit testing, evaluation of risk, status, prescription of WIC food package, documentation, and high-risk referrals following program standards and protocols; provide and document individual nutrition education counseling and follow-up for participants with low risk medical conditions. Clinical Duties:-Plan, conduct, and evaluate group nutrition education classes using the facilitated learning process in accordance with WIC guidelines and agency policies; document all nutrition education contact according to WIC policies and procedures.
-Perform hemoglobin controls as needed; document care provided in accordance with agency policy and or procedure; discuss appropriate findings with a Registered Nurse for review.
-Review immunization status, administer age appropriate vaccines per standing orders. Order, stores and processes vaccines in accordance with the Department of Public Health (DPH) policy and procedures; administer vaccines according to agency policies and as stipulated within the Nurse Practice Act.
-Provide consultation and technical assistance to other members of the multi-disciplinary health care team and community in areas related to normal nutrition, including health promotion and disease prevention; participate as a member of the multi-disciplinary WIC Team in clinic and or class scheduling, caseload monitoring, and outreach. Work with other program staff and disciplines to enhance integration and quality of service.
-Provide assistance to families with infants on contract and List I formulas including education, dispensing, and monitoring as assigned. refer participants on List II and metabolic formulas to Registered Dietician (RD) according to WIC guidelines; follow established guidelines for customer services and cultural competency to include annual review of WIC Civil Rights and Customer Services.
-Provide breastfeeding promotion and support to pregnant and breastfeeding women. Screen and refer pregnant women and infants to the Breastfeeding Coordinator according to the Referral Guideline; issue and monitor breast pumps and breastfeeding supplies.
Emergency Preparedness:Participate in disaster preparedness as outlined by the agency DPH, the region's, and county plan, which is subject to duty and/or call on a 24-hour basis as an essential employee.
This position is located in the York County Health Department. Minimum and Additional RequirementsState Minimum Requirements: Positions in this classification require licensure as a Licensed Practical Nurse by the South Carolina Board of Nursing; Graduation from an accredited school of practical nursing.
