Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, take and monitor vitals of clients on the unit, medication administration and documentation. 2. Renders and supervises care of clients on the unit. 3. Monitors medication and consulation with prescribers. 4. Provide appropriate health care and coordinates referrals as prescribed by providers. 5. Participates in the evaluation of client's condition and progress with team members. 6. Promote education and adherence to prescribed treatment protocols. 7. Facilitates wellness and medically related psychoeducational groups to individuals. 8. Records all client information within electronic medical record system daily. 9. Assists RN on daily basis. 10. Offer nursing consult on emergency service evaluations. 11. Work with clients to coordinate care with their PCP, psychiatrist, and other professionals including but not limited to medication refills and f/u medical appointments. Attends variety of meetings, conferences, seminars as required or directed. 3. Demonstrates use of Quality Improvement in daily operations. 4. Complies with all health and safety regulations and requirements. 5. Position Qualifications: License/Certification/Education: Required: 1. Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts. 2. Satisfactory CORI background check. Preferred: N/A Experience/Skills: Required: 1. Minimum 2 years' experience in medical nursing. 2. Knowledge of medical, substance abuse and mental health nursing. 3. Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies. 4. Demonstrates ability to communicate effectively and professionally with clients, staff, and providers. Preferred: N/A Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments.
