Sapphire Health Services is currently seeking a dedicated Licensed Practical Nurse (LPN) to join our team and uphold our commitment to excellence.

JOB SUMMARY:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our skilled nursing facility. The LPN will be responsible for delivering quality patient care in compliance with facility policies, state regulations, and healthcare standards. This role is crucial in ensuring the health, comfort, and safety of our residents.

RESPONSIBILITIES:

  • Assists with new admissions
  • Provide medical intervention as needed for emergent needs
  • Communicate needs of resident to outside providers including but not limited to: Primary Care Provider, Home Health Agency, Pharmacy, etc
  • Provide direct nursing care to residents, including administering medications, treatments, and monitoring vital signs
  • Perform wound care, dressing changes, and other therapeutic nursing interventions as directed
  • Observe residents for changes in health status and report any concerns
  • Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding
  • Collaborate with interdisciplinary teams to ensure quality resident care
  • Maintain accurate and timely documentation of resident care, treatments, and progress in electronic medical records (EMRs) or paper charts
  • Ensure compliance with all federal, state, and local regulations as well as facility policies and procedures
  • Provide emotional support to residents and their families, fostering a compassionate and empathetic environment
  • Participate in staff meetings, in-service training, and continuing education programs to stay updated on best practices
  • Support infection control and safety protocols throughout the facility
  • Supervise certified nursing assistants (CNAs) and provide direction as needed
  • Other Duties as Assigned

REQUIREMENTS:

  • Current Licensed Practical Nurse (LPN) license in the state of practice
  • Experience in a skilled nursing facility or similar healthcare setting (preferred)
  • Basic Life Support (BLS) certification required
  • Strong understanding of geriatric care and the needs of elderly residents
  • Proficiency in using electronic health records (EHR) systems
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and handle multiple tasks with attention to detail
  • Compassionate, patient-focused, and empathetic approach to care
