WIHD is actively recruiting for a part-time Licensed Practical Nurse (LPN). This is an exciting opportunity for a growing outpatient clinic seeking an LPN to participate in a broad range of activities supporting patients with intellectual and developmental disabilities at a center of excellence in Valhalla, NY. The LPN position is part time, 21-24 hours per week and includes benefits such as Term Life, STD, LTD, Paid Time Off and 403(b).

Responsibilities Include:

Direct Patient Care/Contact

  • Prepares patients for clinical examinations and interventions, as needed;
  • Assists providers in conducting examinations and procedures;
  • Administers medications and immunizations upon request;

Treatment Coordination

  • Facilitates chart preparation including collecting all results prior to the scheduled patient visit;
  • Obtains, as needed, prior authorization for WIHD services and for referrals and prescriptions generated by WIHD providers. Organizes appeals if denied;
  • Facilitates referrals to other WIHD clinical specialties and other agency programs and services;
  • Generates referrals forms for outside services including the collection and submission of supporting documentation required;
  • Support the health of patients by regularly confirming that appointments, test, procedures and forms are appropriately scheduled and up-to-date. Speak with patient and/or caregiver regularly and document discussion in medical notes within EMR;
  • Serves as a liaison with hospitals, physicians offices and third party payors regarding patient care issues and the need for outside tests and procedures;
  • Assists providers in monitoring the results of labs and other tests ordered. Alerts providers to abnormal findings;
  • Obtains information from agencies, pharmacies, etc. required for patient care;
  • Completes paperwork to facilitate surgical coordination and for other purposes such as surrogate court.
  • Provides education for patients, agency staff, caregivers and families regarding clinical recommendations and self-care strategies and documents such activities using the electronic health record.

Other Tasks

  • Assist in keeping EHR charts current including medication reconciliation, test results and consultation notes from outside providers. Utilizing the EHR to its fullest potential;
  • Addresses patient and/or agencies concerns or calls them to the attention of Health Services supervisory staff to address;
  • As requested by RN, responds to emergencies throughout WIHD and stabilizes situation until EMS arrives;
  • When necessary, help medical assistants complete their assigned duties;
  • When necessary, provide administrative coverage and back-up support to other positions in Adult Health to include medical reception desk;

Requirements:

  • Minimum two years recent pertinent and professional clinical management experience;
  • Current NYS LPN License;
  • Knowledge of care delivery systems, and state and federal regulations;
  • Minimum one year experience in direct nursing care to individuals with developmental disabilities is preferred.

Pay: $33.00 - $36.00 per hour
