Licensed Practical Nurse

FULL TIME, FLEX (4 DAYS A WEEK), PRN POSITIONS AVAILABLE

SIGN-ON BONUS OF $6,000 FOR FULL-TIME AND $4,500 FOR FLEX POSITIONS

Landmark Home Health is a non-profit organization dedicated to delivering high-quality home health services to patients in the comfort of their own homes. We are hiring Licensed Practical Nurses in the Monroeville, Penn Hills, Plum, Verona, McKeesport, and surrounding areas. As an LPN at Landmark Home Health, you will play a pivotal role in providing high-quality care to patients. We require strong clinical and computer skills. Home Health Care experience is not required but a PLUS.

We support the Public Service Loan Forgiveness (PSLF). The PSLF program helps individuals in public service roles alleviate the burden of student loans.

RESPONSIBILITIES:
Monday-Friday Position.
Requires one summer and one winter holiday.
Provides nursing care following the established Plan of Care.
Reports clinical findings to primary nurse on a daily basis.
Evaluates the patient's progress.
Informs the physician and other responsible personnel of significant changes in the patient's condition.
Discusses with patient/caregiver any changes in the patient's care plan.
Provides instruction to patient/caregiver regarding disease process, treatments and goals.
Completes patient documentation within the established time frame assuring information is appropriate.

QUALIFICATIONS:
Current Pennsylvania Nursing License is required
Must have one (1) year of current nursing experience preferred
Valid current driver's license and auto insurance coverage is required
