Licensed Practical Nurse

•  Provides therapy, counseling, and intervention services to patients referred by physicians.
•  Conducts individual and family counseling, mental health assessments, and crisis intervention.
•  Collaborates with providers to create and implement treatment plans and makes referrals as needed.
•  Offers both individual and group therapy, including services for co-occurring disorders.
•  Responsible for documentation, case management, and participation in teams and committees.
•  Requires a Master's or Doctorate and relevant Illinois clinical licensure, with clinical experience preferred.
•  Must demonstrate emotional resilience, communication skills, and ability to work with all age groups.
•  Work environment is an office setting with potential exposure to communicable diseases.
