LCSW is an important augmentation of the case management process to support member care needs are identified that require additional support outside of the member's medical needs often focused on their emotional and social needs. LCSW establishes a course of action by exploring options and setting goals with the member, family, or caregiver to ensure member care needs are augmented by community-based services and identification of additional resources by establishing a liaison role between community services and the Case Management team
Licensed Clinical Social Worker shall:
• Counsel on Substance Abuse and rehab strategies
• Coordinate Social Services
• Facilitate care transitions related to behavioral health services including facility admissions, facility admission diversions, discharge to home or other living arrangements, and step down to alternate clinical care setting (i.e., residential treatment, Partial hospital, Intensive Outpatient Treatment
• Work closely with ACM, ICM, and Coordinators to coordinate medical, behavioral, social, and other services including but not limited to LTSS.
