Lead Speech Language Pathologist

•  The Registered Nurse (RN) collaborates with an interdisciplinary team to provide compassionate hospice care addressing patients' physical, medical, psychosocial, emotional, and spiritual needs.
•  Key responsibilities include case management, comprehensive assessment, care coordination, supervision of nursing staff, documentation, patient/family education, and quality improvement.
•  The RN delivers individualized care, participates in team meetings, oversees infection control, and advocates for patient and family needs.
•  Requirements: current RN license, at least two years of recent hospice or home health experience, reliable transportation, and physical ability to support patient needs.
