Job Summary

Develops and maintains collaborative relationships with all network partners and members. Leads projects and initiatives to support the networks contract goals. Supervises daily activities of the practice transformation team.



Responsibilities

Collaboration
Serves as main point of contact for the clinical team with all payers fostering collegial relationships. Collaborates with the clinical team to track issues such as attribution, measure definitions, and payer reports for value -based programs and problem solves with the payer contacts for resolution. Acts as a liaison between the clinical team, data analyst, IT, and vendors to obtain actionable reports. Collaborates with clinical team to develop resources when they do not exist. Collaborates with clinical team, IT, and data analyst to ensure data integrity and consistency from baseline through monitoring and evaluation of outcomes.

Leadership
Acts as the subject matter expert for value-based programs, prepares summaries of programs, and provides education to internal and external customers. Assists the ACMO, Population Health Director, and Manager of Care Coordination for the clinical team to set timelines and priorities to meet contract goals and deliverables Supervision of practice transformation team and assists with daily problem solving and mitigation of barriers. Assumes leadership role in planning, implementation, and evaluating grant-funded and other network projects in collaboration with ACMO and Manager.

Department & Team Development
Identifies practice needs for educational materials for staff and patients and existing resources. Develops training plans for the clinical team to support the network practices in meaningful use to drive improvement. Communicates project goals and outcomes to internal and external customers. Serves as a backup for the population health/practice transformation team to support new network organizations and to orient to the value -based programs and measures.



Knowledge, Skills & Abilities

Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs.

Minimum of five years related experience and supervisory experience required. Knowledge of coding, payer contracting, and quality payment programs preferred.



Education

Bachelor's Degree (Required)
Master's Degree (Preferred)

Required qualifications:
  • 5+ years of experience in the healthcare industry
  • Legally authorized to work in the United States
  • 16 years or older
  • At least bachelor's degree or higher
