Job Summary

We are looking for compassionate professionals with a background in social or human services to become IRIS Customer Support Consultants. This remote position allows you to build lasting relationships and support individuals enrolled in the Wisconsin IRIS Program.

Your role involves partnering with IRIS participants in their homes and communities, helping them navigate the program and achieve their long-term care goals. You will connect individuals to vital resources and develop tailored plans for achieving outcomes related to employment, housing, health, and community engagement.

Your expertise will help participants understand their rights and responsibilities, ensuring they can make the most of their IRIS benefits. As an IRIS Consultant, you will experience diversity in your daily tasks while providing invaluable support to those in need.

Key Responsibilities
  • Conduct a minimum of four in-person meetings per year with IRIS participants, including one required home visit annually.
  • Provide orientation to new participants, clarifying their rights and responsibilities within the program.
  • Explore each participant's life goals, interests, and community connections to develop effective, customized IRIS plans.
  • Assist participants in achieving personal goals within their budget, documenting activities and changes in the IRIS data system.
  • Research community resources to support individual outcomes and share valuable information with participants.
  • Educate participants on budget management and provide support in navigating financial relationships with service providers.
  • Foster strong, trusting relationships with participants, ensuring clear communication about program changes and compliance.
  • Maintain confidentiality and follow HIPAA regulations in all interactions.
  • Collaborate with other team members to implement participants' plans successfully.
  • Participate in monthly team meetings and weekly check-ins with supervisors for ongoing support and development.
Qualifications

Education: Bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a related field; or four years of relevant experience.

Experience:
  • At least 1 year of direct experience in social service delivery for individuals with disabilities or older adults.
  • Ability to work independently and remain self-motivated.
  • Knowledge of long-term care programs and principles of self-determination.
  • Strong problem-solving and critical thinking skills.
  • Excellent time management and prioritization capabilities.
  • Ability to develop and maintain professional relationships.
  • Resourcefulness in identifying community resources and supports.
  • Proficient with technology, including Microsoft Office Suite and online databases.
Additional Requirements: A valid driver's license and reliable transportation are necessary for in-person meetings.

Pay Range: $18.04 - $35.17 / HOURLY
