DESCRIPTION:
- 4 days per week
- 3 clinic days and 1 nursing home rounds and visits
- Varied shifts available
- Credentialing approximately 30-60 days
- Nursing home schedule incorporated into the clinic schedule
- Will participate in coverage for the Palative care team for inpatient services
- Staff; Office manager, full time RN coordinator, 2 full time RN, full time LPN, office scheduling assistant
- Call; 10 days per month including one weekend is expected
