Clinical Performance: Provides safe therapeutic care in a holistic and systematic way using a relationship-based care approach. The needs of patients and families across the continuum are identified and met and includes opportunities for patient and family education. Evaluates the effectiveness of self and care given by all health care team members in progressing patients toward goal. Demonstrates knowledge of expert resources and support services that are available and how to access them when needed. Patient Safety: Situations are analyzed for potential safety concerns and appropriate action is taken to reduce or eliminate risks for potential harm. Exercises good judgment and knows when to seek outside advice when solving problems to prevent unfavorable patient outcomes. Maintains up-to-date knowledge of policies, procedures, code of conduct, safety protocols, and infection prevention standards, and follows them accordingly. Education: Maintains current knowledge of nursing information both in general and specific to the department. Attends all classes as required and seeks out opportunities to learn and grow. Completes all mandatory education in a timely manner. Documentation: Documents patient care as required by hospital policy and department standards in a timely manner. Medication: Administers medication according to the 5 Rights and hospital policy. Educates patients to the action, benefits, and side effects. Monitors for adverse reactions. Develops plan of care that is individualized for the adult or geriatric patient reflecting collaboration with other members of the healthcare team. Conducts ongoing assessments as determined by the patient's condition, policy and procedure, or protocols and reprioritizes care accordingly. Safely prepares, administers, and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with policy. Initiates patient education and the discharge plan by teaching patients and significant others how to manage their illness/injury, signs of complications, contact information and referrals to other healthcare professionals for follow-up treatment. Ensure equipment or devices are working properly, cleaned, and stored after use. Adheres to infection prevention practices and facility safety policies and procedures
