Specific Responsibilities:

This is a Human Services Program Analyst - SES position responsible to direct and support Family Support Workers (FSW). Work is performed under the supervision of the Community Health Nursing Consultant. This is a bilingual position fluent in English/Spanish or English/Creole. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating, motivating, training, and evaluating employees in addition to planning and directing employees' work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets. Adherence to all established Florida Department of Health Safety Procedures and Practices while in the workplace and in the community.

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures and Children's Services Council (CSC) Information Security Policy, Protocols and Procedures. Confidential data sets: Has access to include Clinical Information, Health Management System (HMS)-Employee Activities Record (EARS), and the Healthy Beginnings Data System (HBDS) for the purpose of running reports, entering and reviewing client data, and uses an electronic medical record database.

Coordinates visits between nurse home visitors, FSWs and program participants who require medical interpretation services. Conducts field observations of client encounters as recommended by the Healthy Beginnings System of Care. Monitors team productivity. Evaluates face-to-face services, and reviews and approves staff travel mileage expenses. Completes second-level screenings of non-English speaking HBDS client referrals. Assesses for language skills, understanding of program services and level of commitment to enroll in program services. Communicates outcomes with nursing supervisor supporting the rapid disposition of the referral. Participates in trainings and meetings as required by the FDOH, and the Children's Services Council (CSC) of Palm Beach County. Creates strong working relationships to accomplish goals of the Agency, and the Healthy Beginnings System of Care. Ensures staff attend Healthy Start, CSC mandatory education sessions and maintains training records.

Participates in continuous quality improvement. D evelops tools to measure the quality of services. Responsible to create an electronic survey, input, update, monitor, trouble shooting and complete mandatory Healthy Start Survey in the Healthy Beginnings Data System (HBDS) and Department of Health SharePoint web-based for all prenatal and infant clients in the Healthy Beginnings Nurse (HBN) Program and the Women's Health Initiative Program (WHIN). Completes and submit required report to program consultants. Coordinate staff to support in person completion of surveys for clients who are not able to complete survey electronically. Assesses and supports the professional development of staff by identifying strengths and areas for professional growth. Collaborates with community agencies. Coordinates pick-up and distribution of items designated for program participants from community partners.

Provides programmatic reports and input to the Community Healthy Nursing Consultant for strategic planning of the program. Assists in planning and conducting team meetings, team education, client case conferences, and staff meetings. Orients new staff. Develops written procedures, protocols, processes, worksheets and reporting tools as assigned.

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Valid Driver's License.
  • Travel is required in order to perform the duties of this position.
  • This position works primarily in an office setting and involves sitting for long periods of time, and the moving, lifting, pushing and carrying objects up to 25 lbs., multiple times throughout the day, and occasionally working in a community setting providing home visits and involves getting in and out of a vehicle, walking up and down stairs.
