Job Purpose

A) The position of HUD case manager is as a credentialed Qualified Mental Health Professional-CS. It is a direct care position with the expectation that at least 50% of his/her work time is spent face to face with the patients. Focusing on provision of ongoing services to assist patients who are homeless and has chosen to enroll in an MHMR Tarrant supportive housing program.

B) This position specializes in providing services for the purpose of empowering patients to achieve goals set forth in their wellness plan and become self-sufficient to maintain the patient's housed status using the Housing First model.

C) Case manager provides assessments, wellness planning, and ongoing treatment in order to assist patients with severe and persistent mental illness and/or substance abuse diagnosis to gain the skills, resources, and supports necessary to attain their individualized recovery goals including access to an acquiring medication, employment assistance, assistance in acquiring disability benefits, resolving legal issues, and any other barriers that may exist.

D) Services may be provided in the clinic or in the community. Providing services in the community entails travel (often in the case manager's own vehicle) and work throughout Tarrant County, in a variety of neighborhood and home conditions.

Minimum Education: Bachelor's Degree in a Health or Human Services

Acceptable Degrees Include: Applied Behavior Analysis, Behavioral Services field, Communication Disorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health Services, Human Services field, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services field, Social Work, Sociology, and Special Education

Preferences: Experience in the treatment of substance abuse or behavioral health

Substitutions: May substitute a bachelor's degree with at least 30 hours of behavioral health coursework which must be documented within the transcript.

Years' Experience: One-year full work experience in the social, behavioral or human services field preferred, but not required.

License/Certifications : Valid Texas driver's license with an acceptable driving record
