The Housing Coordinator will coordinate housing efforts for the HUD and PATH PSH housing programs by developing and maintaining relationships with All Chicago, managing entity for the CoC, to support the program with Coordinated Entry System (CES) and Homeless Management Information System (HMIS) related navigation. They will collaborate with different teams/programs, and with representatives from the city, state, and county to secure safe and affordable housing for clients. The Housing Coordinator will support in managing all aspects of the United States Department of Housing and Urban Development (HUD) grant and IDHS grants for the programs. They will be responsible for matching clients on the CES list to the program and ensuring all documentation is available upon move in. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance use utilizing a harm reduction approach. The Lead Housing Coordinator will represent Trilogy at Continuum of Care meetings and other assigned state and local events and committees as needed.

Responsibilities:
  • Manage aspects of the HUD and IDHS grants, i.e. identify participants within the CES, manage participants to successfully maintain housing in accordance with the housing first model of care, database management, etc.
  • Member of the Continuum of Care (CoC) committee to help meet requirements of the HUD grant. Actively participates in CoC meetings for the organization.
  • Support clients in locating and securing housing in the community.
  • Communicate with landlords and property managers to discuss subsidized housing programs.
  • Meet with clients to provide direct and supportive services related to housing, i.e., lease negotiation process, move-ins, move outs, and ensure clients are linked to intake for completion of intake paperwork and IMCANs within first 30 days.
  • Prepare and report information in the HMIS system to ensure the department is meeting benchmarks and standards for grant programs.
  • Expert knowledge and ability to navigate the Coordinated Entry System (CES) and the Homeless Management Information System (HMIS) and manage all referrals and for HUD, PATH Permanent Supportive Housing, and Coordinated Entry matches.
  • Support in setting up client apartments to prepare for move in.
  • Provide effective and collaborative communication with external and internal programs to support a smooth process of record gathering to ensure new clients are document ready when moving into the program.
  • Communicate and collaborate with both internal and external programs and community outreach teams to enhance client care, service delivery, and facilitate program operations and objectives.
  • Perform other related duties and/or projects as assigned.
Qualifications:
  • Must have one of the following requirements:
    • High School Diploma or GED.
    • Associate or bachelor's degree in any field.
  • At least 2 years of related experience preferred.
  • Valid Illinois driver's license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance
