Summary We are seeking a dedicated and compassionate Housing Case Manager to join our team. The Housing Case Manager will play a crucial role in supporting individuals and families who are experiencing homelessness by providing housing placement services and supportive care. This position requires a proactive approach to housing placements, client advocacy, and comprehensive service coordination. Key Responsibilities - Housing Placement
- Complete the admission process for all new Permanent Supportive Housing (PSH) participants.
- Verify homeless eligibility of all PSH participants and conduct initial needs assessments.
- Provide transportation, bus vouchers, and ensure immediate needs (food, clothing, hygiene products) are met for new PSH participants.
- Complete household assessments, document homelessness, and develop service plans for eligible clients.
- Assist with housing search and placement, including landlord negotiations and lease guidance.
- Perform HUD home inspections and ensure leasing requirements are met.
- Provide moving, furnishing assistance, referrals, and guidance.
- Supportive Services
- Help clients secure mainstream benefits (food stamps, SSI/SSDI, VA benefits, medical insurance, etc.).
- Assess medical and health needs, making appropriate referrals for care.
- Offer employment and vocational service referrals.
- Provide supportive counseling and connect clients to community resources.
- Document all services in the Homeless Management Information System (HMIS) within 24 hours of service delivery.
- Maintain SSI/SSDI Outreach, Access, and Recovery (SOAR) certification.
- Perform any other duties assigned by the supervisor.
Job Qualifications - High school diploma/GED required, with two years of service coordination or related social service experience. A bachelor's degree is preferred.
- Knowledge of homelessness and housing programs is highly beneficial.
- Experience working with individuals and families experiencing homelessness.
- Familiarity with de-escalation techniques, harm reduction strategies, and crisis intervention.
- Exceptional problem-solving, communication (both oral and written), and organizational skills.
- Proficiency in standard software applications and the ability to manage digital files and navigate the internet.
- Valid LA driver's license and access to an insured motor vehicle to transport clients.
Job Specifications - Medium: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. 100% of the time.
