Housing Case Manager

The primary responsibility of this role is to deliver comprehensive care and support to enhance the well-being of service recipients. This involves providing therapeutic support and skills training both in person and virtually, fostering communication with clinical professionals and the individual's support system as authorized. Acting as a role model and mentor, the position includes coaching service recipients using approved techniques and strategies and implementing physical intervention when necessary to ensure the safety of individuals. Additionally, meticulous completion of documentation and reporting for grant programs is essential to contribute to the team's operational effectiveness.

Minimum qualifications: High School Diploma

Preferences: bachelor's degree in human services

Substitutions: 60 college credit hours with two years-experience working with IDD or behavior health needs

Years' Experience: Two (2) yearsWorking with individuals with IDD or Behavior Health needs

License/Certifications: valid TX DL
