.The Opportunity We are seeking a self-motivated, compassionate individual to fill the role of Housing Case Manager. The Housing Case Manager provides a comprehensive assessment of clients requesting social service assistance. They are responsible assessing client needs and providing relevant resources through one-on-one interviews. The position requires advocacy and collaboration with clients, service providers, and agencies. The Housing Case Manager designs, recommends, implements, and evaluates strategies that help move families out of poverty using a bundled approach (Working Family Success) and engages, monitors, and coaches clients using techniques that will lead to the client's financial success.
Requirements
Minimum Qualifications - Bachelor's degree in Social Work or related field
- Minimum two years of experience working with housing instability and case management for families and those at risk for homelessness
Preferred Qualifications - 3 years experience in a non-profit setting
- Bilingual - English/Spanish
Major Responsibilities & Duties - Schedule and conduct individual (virtual/in-person) client appointments using motivational interview techniques in determine clients' housing options and needs based on crisis
- Develop individualized case management plans that drive our clients towards self-sufficiency
- Complete the eligibility assessment and verify documentation
- Prepare and submit all housing related payment documents for clients
- Maintain accurate and confidential records for each assigned client
- Maintain program policies and guidelines for all clients
- Maintain open communication with other program staff
Special Knowledge/Skills - Demonstrated ability to learn new processes through self-study or practical experience
- Ability to advocate for clients' needs, coordinating services with community resources, and maintaining accurate case records.
- Knowledge of the barriers facing families/individuals experiencing housing instability or homelessness (including family dynamics, legalization statues, employment/financial instability, mental illness and substance abuse)
- Knowledge and effective execution of de-escalation techniques and conflict resolution
- Knowledge of Salesforce, HMIS or related client management databases
- Ability to work with a diverse client base and a multi-disciplinary team
- Ability to establish and maintain effective working relationships with team members, guests, and the general public
- Excellent communication, organizational and time management skills with ability to perform duties with minimal supervision if necessary

Working Environment/Physical Requirements - Work is primarily performed in a climate-controlled office environment, with occasional outdoor work (rate and typically related to a voluntary event)
- Workspace includes adjustable, electronic standing desk
- Must be able to push, pull, lift, and place up to 20lbs without assistance
- Hand use: single grasping, fine manipulation, pushing and pulling
Reporting Relationship The Housing Case Manager reports directly to the Manager of Housing and Senior Services.
Pay Range The Housing Case Manager is a full-time, FLSA exempt position with a starting annual salary range of $48,000 to $49,250 (Depending on Qualifications).
