Job Details

We are seeking a compassionate and dedicated Case Manager to join our client's team. You will work closely with clients to assess their needs, create individualized plans, and coordinate with community resources.

Responsibilities:
  • Assess client needs and develop personalized service plans.
  • Connect clients to housing, employment, health, and social services.
  • Provide emotional support and guidance during difficult transitions.
  • Document case progress and update records in compliance with program standards.
  • Collaborate with community partners to ensure clients receive comprehensive care.
Qualifications:
  • Experience working with underserved populations, preferably individuals infected or affected by HIV/AIDS.
  • Bachelor's degree in Social Work, Psychology, or a related field preferred.
  • Strong communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Empathy, patience, and commitment to making a difference.
