Job Details We are seeking a compassionate and dedicated Case Manager to join our client's team. You will work closely with clients to assess their needs, create individualized plans, and coordinate with community resources.
Responsibilities: - Assess client needs and develop personalized service plans.
- Connect clients to housing, employment, health, and social services.
- Provide emotional support and guidance during difficult transitions.
- Document case progress and update records in compliance with program standards.
- Collaborate with community partners to ensure clients receive comprehensive care.
Qualifications: - Experience working with underserved populations, preferably individuals infected or affected by HIV/AIDS.
- Bachelor's degree in Social Work, Psychology, or a related field preferred.
- Strong communication, organizational, and problem-solving skills.
- Ability to work independently and as part of a team.
- Empathy, patience, and commitment to making a difference.
