About the Role
Provide comprehensive primary care services for patients in a short term rehabilitation and long term residential facility.
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Practice at the top of your license with autonomy
• Ensure smooth transitions of care with hospice, palliative care, and home care agencies

• Graduate degree as a nurse practitioner or physician assistant
• Active and unrestricted NP or PA license in North Carolina
• Strong clinical assessment and diagnostic skills
• Excellent communication and interpersonal abilities
• Experience in post-acute care preferred, but not required (training available) Our compensation model is a fee for service model with an expected compensation estimated range of $103,000 to $125,700 annually with no cap on productivity income.
