Our mission is to provide optimal comfort and dignity to our patient through their end-of-life journey. We will achieve this by concentrating on patient and family-centered goals focusing not only on the medical needs but also on providing care for the mental and spiritual needs of our patients and those they hold dear.

Hospice Philosophy:

Hospice care is an interdisciplinary approach designed to meet the needs physical, psychological and spiritual of a person at the end of life with the goal of comfort and not cure. Hospice considers the unit of care to extend beyond the patient and include the family and caregivers as defined by the patient.

Being a hospice volunteer is an individual trained to provide assistance and companionship to our patients and their family.

Heart 2 Heart Hospice Care is a locally owned and operated, boutique hospice servicing the Portland Metro area. With over 15 years of hands-on senior care experience, our team understands the needs, challenges and regulations communities and medical professionals face, which is one of the many attributes differentiating us from the other hospice options. Our goal is to provide the highest level of hospice care to patients, families and providers which includes personalized care and crisis support
