Compensation: Pay per visit plus admin hourly rates, processed bi-weekly.

Shift Type: Full-time, Monday-Friday, 8/9 AM to 4/5 PM (flexible).

Summary
We are looking for compassionate Registered Nurses (RNs) to work one-on-one with our patients and oversee all aspects of case-managing patient care. Case management involves creating a Plan of Care for patients and following the physician's orders. This role is essential in coordinating patient care, ensuring that individuals receive the appropriate services and support throughout their healthcare journey. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care.

Responsibilities

Assess and evaluate patients' needs.

Develop and implement individualized care plans in collaboration with healthcare providers.

Coordinate patient services across various departments, including primary care and specialty clinics.

Serve as a liaison between patients, families, and healthcare teams to ensure effective communication and understanding of treatment plans.

Monitor patient progress and adjust care plans as necessary to meet evolving health needs.

Educate patients and families about health management, treatment options, and available resources.

Maintain accurate documentation of patient interactions and care plans while adhering to HIPAA regulations.

Participate in quality improvement initiatives to enhance patient outcomes.

Qualifications

Minimum of 1 year of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.

Active State Registered Nurse licensure is required.

Proof of current CPR certification and valid Driver's license.

Active auto insurance coverage is required.
